The Student named on the registration form has my permission to participate in classes and/or lessons at the Musical Arts Center. I understand and accept the policies of the Musical Arts Center as stated on this website and accept responsibility for charges and fees incurred. I understand that MAC policies are subject to change and notification will be made by letter and/or website. I will allow the Center to use photographs and recordings made at the Center or at Center functions involving the student hereby enrolled.
